29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section: a) Job site, 269 Curie Drive, Portersville, PA: On or about April 26, 2023, during service/maintenance activities on a mechanical tarp system on the passenger side of a slinger truck, the employer did not ensure that machine specific energy control procedures were documented and utilized. An employee worked on a damaged tarp system and the kinetic energy stored in it's spring system was not controlled, causing the stored energy to be released, resulting in an employee falling and sustaining a fatal head injury.
