29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. a. The employer did not report an employee fatality within 8 hours of being aware of the fatality. On or about April 20, 2023, an employee was fatality injured by gunshot wounds.
