29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): On or about April 19, 2023, the employer failed to ensure that each employee is protected from falling through a gap between the Saferack G4 Series Gangway's guardrail system and/or a hole opening at the top of the tankard.
