29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Tennessee Business Forms dba PrintXcel, located at 4444 N. Detroit Ave, Toledo, Ohio 43612: On or about April 13, 2023, employees operating the Mega Collator during production were exposed to caught in hazards in the Processing Section of the machine. Employees were exposed to in-running nip points in the Numbering section, Crimping section, Slitter section, and Delivery Section in that no guarding was provided to prevent employees from entering or inserting body parts into the machine.
