Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and caught-in hazards: (a) On or about April 12, 2023, and at times prior, employees were assigned to inspect the trolley return chain system for trolleys that had fallen from the chain and rail onto the tray and retrieve the trollies from the trays. To see and reach the inside of the tray and access the rail and chain, employees stood adjacent to the trolley chain system in a locations where trolley hooks could swing, catching the workers. Workers were exposed to struck-by and caught-in hazards with no means to stop the trolley return chain. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install estops or an estop cable where workers are exposed to moving parts of the return trolley system in accordance with the guidelines set forth in NFPA 79.
