29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 10 April 2023, an employee for Cheltenham Construction Services, Inc. dba CCS was exposed to a fall hazard at 521 Greengage Timber Drive in O'Fallon, Missouri. The employee was installing siding on a portion of the second story of a residential building. The employee accessed the roof using a ladder that was not extended 3 feet beyond the upper landing surface.
