29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: 438 S. Callaghan Rd, San Antonio, TX.: On or about March 25, 2023, and at times prior thereto, employees performing heat unit work to an electrical room, were exposed to fall hazards while using a walk platform with missing grates.
