29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On or about March 22, 2023, at an establishment located at 3243 Whiting Road, in Stevens Point, Wisconsin; in the Stock Prep Area, a Toyota #93 forklift was utilized to lift and maneuver cottons bales. The forks were elevated when the final steel band on a stack of three high cotton bales was cut resulting in the bales falling and striking an employee.
