29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment: Sofo Foods of Ohio, LLC - Toledo, Ohio: a) On or about March 29, 2023, the employer failed to ensure that written procedures were developed and implemented for inspection, testing and maintenance of process equipment, including compressors, such as the RC-1 Compressor associated with the ammonia refrigeration system at the site. Failure to fully implement inspection and testing procedures can expose employees to hazards including but not limited to inhalation of ammonia, and fire/explosion hazards in the event of an ammonia release event. b) On or about March 29, 2023, the employer failed to ensure that written procedures were developed and implemented for inspection, testing and maintenance of process equipment, including condensers, such as the EC-2 Condenser associated with the ammonia refrigeration system at the site. Failure to fully implement inspection and testing procedures can expose employees to hazards including but not limited to inhalation of ammonia, and fire/explosion hazards in the event of an ammonia release event. c) On or about March 29, 2023, the employer failed to ensure that written procedures were developed and implemented for inspection, testing and maintenance of process equipment, including pressure vessels, such as the PV-1 Pressure Vessel associated with the ammonia refrigeration system at the site. Failure to fully implement inspection and testing procedures can expose employees to hazards including but not limited to inhalation of ammonia, and fire/explosion hazards in the event of an ammonia release event. d) On or about March 29, 2023, the employer failed to ensure that written procedures were developed and implemented for inspection, testing and maintenance of process equipment, including pumps, such as the RP-1 pump associated with the ammonia refrigeration system at the site. Failure to fully implement inspection and testing procedures can expose employees to hazards including but not limited to inhalation of ammonia, and fire/explosion hazards in the event of an ammonia release event.
