29 CFR 1910.119(d)(3)(i)(B):The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process. a) 66 Kettles process area - On or about March 27, 2023 the employer did not ensure that the piping and instrument diagram (P&ID) accurately represented the existing equipment on reactor 4A of the 66 kettles process in that rupture disk PSE 820-009 was listed on the P&ID as being rated for 45 PSI at 100 Celsius, whereas the rupture disk and other design documents report it being rated for 45 PSI at 72 Fahrenheit. Failure to provide accurate P&IDs may potentially cause incorrect operations or maintenance. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
