OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in / crushed by hazards during operation of a plastic film laminating machine. a) Coater #5 Accumulator: Employees were exposed to crushed by and caught in hazards from not following safe methods of operation, including, but not limited to, proper use of machine controls, proper use of the interlocked safety gate, and the proper personal protective equipment and clothing for the task at hand, on or about 3/23/2023.
