1910.244(a)(2)(iii) Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack: The employer failed to protect an employee from struck-by and crushed-by hazards during maintenance operations on a trailer. The employee sustained fatal injuries when an axle that was not properly supported fell on their chest. The axle was not blocked, cribbed, or chained; jack stands were not used to support/secure the heavy axle to prevent it from falling/dropping onto the employee while working underneath the axle/trailer.
