29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Location: Tool Room On or about March 16, 2023, a pneumatic gravity drop hammer with a continuous airline had no valve to release air during lockout/tagout and a Lockout/Tagout procedure was not developed or implemented exposing employees to crush injuries, fractured bones, and amputations.
