29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) 7201 East Quincy Avenue, Denver, CO 80237: On or about March 15, 2023, and at times prior, employees were allowed to work inside the unprotected trench that was vertical wall of type B soil, approximately 11 feet deep by 8 feet wide, sloughing soil, and excessive water accumulations from a broken sewer line. This condition exposed employees to struck-by and crushing hazards. The employer did not suitably train employees on hazard identification and the methods to control or eliminate the hazards related to trenching operations.
