Dock 11 - On or about March 6, 2023, a contractor employee was permitted to stand on the forks of a Crown Forklift, Model 5700, Serial No. 10493385, while being lifted approximately 9 feet, to repair a dock door. A safe place to ride was not provided, thereby exposing the contractor to fall hazards.
