29 CFR 1904.39(a)(1): The employer did not report the death of an employee resulting from a work-related incident within 8 hours of learning of the death: On or about March 13, 2023, an employee was fatally injured while working at Jarvis Timber LLC. The employer failed to report the fatality to OSHA within eight (8) hours of being notified of the fatally injured employee. OSHA was not notified until March 14, 2023, outside of the 8-hour reporting requirement.
