29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) yard outside machine shop: On or about February 23, 2023, a large valve was rigged with the loops of the lift strap attached to the fork tine and the operator was not trained or certified by a competent trainer, exposing employees to struck by hazards.
