29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, shall be conducted for the operator has been involved in an accident or near-miss incident. Employees were engaged in warehouse activities at a shipping and receiving facility located at 850 Rundell Road, Gering, NE 69341, and were exposed to struck-by hazards in that employees who operate powered industrial trucks, and have been involved in near-misses and accidents, were not retrained and evaluated after these incidents.
