29 CFR 1926.95(b):Where employees provide their own protective equipment, the employer did not assure its adequacy, including proper maintenance, and sanitation of such equipment: 89 Powder Mill Rd. Southwick, MA 01077: On or about March 8, 2023, an employee was assigned to work on a residential rooftop and exposed to a fall height of 9 feet 5 inches while wearing a self-owned Personal Fall Arrest System (PFAS) with a dee-pad that was broken in two places and unserviceable.
