29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about February 17, 2023 at 3665 East 78th Street, Cleveland, OH 44105, the employer failed to ensure employees performing cleaning functions on a full revolution press to clean scale off of the die were protected from crushing hazards in that they did not lock out the press nor utilize a safety bar while performing this task.
