29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about March 2 , 2023, during the installation of roofing material for the construction of new commercial building located at 38872 Mentor Ave. Willoughby, OH 44094, the employer did not ensure employees working from a steep sloped (8/12 pitch) roof were protected from a fall of approximately 25 feet by the use of a guardrail system, safety net system, or personal fall arrest system.
