29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about February 28, 2023, the employer did not ensure that employees performing masonry work at 2576 Kemper Drive, Shaker Heights, OH, 44118 while working from the boom or basket of an aerial lift were protected from falling a distance of approximately 15 feet to the ground below. No form of fall protection was worn by employees.
