29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation, of a powered industrial truck, shall not be performed by the customer or user without the manufacturer's prior written approval. Shipping/Loading Area: On or about February 20, 2023, and at times prior thereto, employees were exposed to a struck-by hazard while using an unauthorized-spreader attachment when loading metal mesh with a forklift into a commercial flatbed truck.
