29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) On or about February 13, 2023, a 7500 pound steel rod was loaded onto the forks of a Hyster 120 forklift by overhead crane. The steel rod was not secured in place on the forklift lifting forks to prevent rolling or becoming unbalanced and falling from the forklift forks, exposing employees to a crushing hazard.
