29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Ivex Protective Packaging Inc. on or about February 14, 2023, at the address of 2600 Campbell Rd Sidney, OH; The employer did not ensure that energy isolating procedures for the Slitter Machine were implemented prior to employee performance of setup activities, such as, but not limited to, weaving foam through the machine and under the cutting blades. Employees performing this servicing and/or setup activities in an energized state were exposed to caught-in hazards. b) Ivex Protective Packaging Inc. on or about February 14, 2023, at the address of 2600 Campbell Rd Sidney, OH; The employer did not ensure that energy isolating procedures for the Slitter Machine were implemented prior to employee performance of setup activities, such as, but not limited to, taping and tucking the foam around the mandrel. Employees performing servicing and/or maintenance activities in an energized state were exposed to caught-in hazards. c) Ivex Protective Packaging Inc. on or about February 14, 2023, at the address of 2600 Campbell Rd Sidney, OH; The employer did not ensure that energy isolating procedures for the OC Hotwire Machine were implemented prior to employee performance of servicing and/or maintenance activities. Employees performing servicing and/or maintenance activities in an energized state were exposed to electrocution and burn hazards.
