29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) On or about 01/30/2023, on the 5:12 pitch roof, employees performing roofing work were exposed to a 21 feet 8 inches fall hazard in that more than one employee was attached per anchor point.
