29 CFR 1910.146(c)(9)(i): Each contractor who was retained to perform permit space entry operations did not obtain any available information regarding permit space hazards and entry operations from the host employer: a) Baghouse, hopper; On or about November 16, 2022 and at times prior to; AHR Metals, Inc, the controlling employer sub-contracted Vallejo's Welding and Industrial Services, LLC to conduct welding work inside of a permit required confined space without first obtaining any available information regarding permit space hazards and entry operations exposing the sub-contractor employees to amputation hazards.
