29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On January 10, 2023, the employer did not ensure the point of operation on the PF SF Dowel, Side Frame Form & Welding Machine, located in the warehouse manufacturing area, was properly guarded in that the operator was using the Mesh Shear Cutter when she sustained multiple finger amputation injuries. This condition exposed employees to amputation hazards. (b) On or before January 11, 2023, the employer did not ensure the Mesh Shear Cutter on the PF SF Dowel, Side Frame Form & Welding Machine, located in the warehouse manufacturing area, was properly guarded to prevent employees from having any part of their body in the danger zone during operation. This condition exposed employees to amputation hazards.
