29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced. Building 13026 Maintenance Bay: On and/or before January 10, 2023, employee(s) used a damaged step ladder to access a lifted vehicles elevated position that was on an alignment rack to perform automotive work. The step ladder's second step was crushed by the alignment rack and was not tagged, repaired, or removed from service. This condition potentially exposed employee(s) to mechanical failure, fall, and/or trip hazard(s).
