OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from crushing hazards to employees due to collapse or tip-over of a storage rack system: (a) Storage Warehouse, Building 666: On or about December 13, 2022, Employees used forklifts to load and unload material on the storage rack system. Employees were exposed to crushing hazards from a collapse or tip-over of a storage rack system due to unanchored rack columns that affected the stability of the storage rack system. The storage rack system located in middle of the warehouse had unanchored rack columns. Abatement Note: Among other methods, feasible and acceptable abatement methods to correct this hazard are to follow American National Standard Institute (ANSI) and Rack Manufacturers Institute (RMI)-ANSI-RMI MH 16:1-2012, "Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks." Under paragraph 1.4.7 Column Base Plates and Anchors: "The bottom of all columns shall be furnished with column base plates, as specified in 7.1. All rack columns shall be anchored to the floor with anchor bolts, which shall be designed in accordance with Section 7.3, to resist all applicable forces as described in Section 2.1 or Section 2.2."
