29 CFR 1910.95(h)(4): Audiometric examinations shall be administered in a room meeting the requirements listed in Appendix D: "Audiometric Test Rooms." On or about December 13, 2022, the employer failed to conduct a Sound Level Survey to determine that the Superior Conference Room and the Office Room on the second floor of the facility fit the Sound Pressure Levels as outlined in Appendix D to determine if the rooms were appropriate Audiometric Test Rooms for audiograms taken on November 16, 2022. Without a survey of the room, audiograms taken in the rooms could be affected by outside noise potentially missing employees that may have developed Standard Threshold Shifts.
