29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 17, 2022, and at times prior thereto, an employee working in work zone was not provided hazard recognition instruction and how to avoid unsafe conditions at the jobsite. Thus, exposing the employee to struck-by hazards. The Perez Trucking was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.21(b)(2), which was contained in OSHA inspection number 1575938, citation number 1, item number 1, and was affirmed as a final order on 7/29/2022, with a final abatement date of 7/28/2022, with respect to a workplace located at 2000 SH-12 Loop N, IRVING, TX 75061.
