Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to under-ride hazards: (a) Warehouse - Sufficient safe clearance did not exist in floor level storage rack slots where employees operated Raymond 7000 series reach trucks. These powered industrial trucks had an operator compartment height above the floor of 51 inches. Some storage racks' first horizontal rack beams had a height above the floor of 60 inches, including, but not limited to, Aisle 40 slot location 42-01-01. This would allow such horizontal rack beams to enter the operator compartment (an under-ride situation), pinning the powered industrial truck operator between the beam and the operator compartment or other truck component, on or about November 30, 2022. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
