Text For Citation: 01 Item/Group: 001 Hazard:

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards while using manlifts: a) On or about November 21, 2022 in Unit 1/2, employees used the manlift carrying freight in buckets between their feet and used the manlift to carry freight exposing employees on lower levels to be struck by falling materials and equipment. b) On or about November 23, 2022 in Unit 1/2, employees used the manlift without a conical guard or floating type guard with a safety switch on the ascending side exposing employees to being struck by the edge of the floor opening. c) On or about November 23, 2022 in Unit 1/2, first floor, employees used the manlift to gain access to upper levels of the mill. Vertical clearance was less than 7-feet 6-inches. d) On or about November 21, 2022, in Unit 1/2, employees using the manlift on the ascending side while wearing backpacks were exposed to being caught in or between the edge of opening and a rising step. e) On or about November 23, 2023, at the first floor of Unit 1/2 manlift, the landing space for ascending travel had a clearance of approximately 17-inches. Employees using the manlift were exposed to being caught between a rising step and a pillar.
