29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that mid rails are installed at a height midway between the top edge of the guardrail system and the walking-working surface. Employees engaged in grain operations at 28115 158th Street, Duncan, Nebraska 68634, were exposed to fall hazards on the southwest upper platform in the loadout area. There was not a mid rail installed between the top rail and the toe board on the backside of the upper platform.
