(a) This most recently occurred on or about Oct 3, 2022, during store operating hours in the employee restroom area of Dollar General Store No. 20469, located at 100 North Menomonie Street, Ridgeland, WI 54763; the employer does not ensure employees are protected from potential injuries from fire or other emergencies when obstructing the exit access route from the employee restroom with shelving units containing various items.
