29 CFR 1910.178(l)(4)(ii)(B):Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident. On or about September 30, 2022, forklift refresher training was not provided to a forklift operator who was involved in a near-miss incident. This condition exposes employees to a struck-by hazard.
