At 4501 Dignan St, Jacksonville Florida: On or about September 21, 2022, the employer exposed ammonia system operators to an egress obstruction hazard in the event of fire or ammonia release in that the ammonia engine room exit door did not have a panic bar release as required by ANSI/IIAR-2, Standard for Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems, paragraph 6.3.1.5.
