29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: At a manufacturing facility located at 1811 E 267th Street, Harrisonville Missouri, on or about September 8, 2022, the employer did not ensure that all forklift operators had a current certification and evaluation to operate the forklift in a safe manner thus exposing employees to struck-by hazards associated with working on and around forklifts
