29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about September 8, 2022, employees were exposed to a fall of approximately 20 feet while performing roofing activities on a residential home located at 1472 W. Clifton Blvd., Lakewood, Ohio 44107. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection. All Sides Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1268494 citation number 1, item number 1 and was affirmed as a final order on 12/18/17, with respect to a workplace located at 1722 Riverside Drive, Lakewood, Ohio 44107. All Sides Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1486038 citation number 2, item number 1 and was affirmed as a final order on 10/1/20, with respect to a workplace located at 2038 Bunts Road, Lakewood, Ohio 44107. All Sides Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1489122 citation number 1, item number 1 and was affirmed as a final order on 10/1/20, with respect to a workplace located at 12541 Lake Ave., Lakewood, Ohio 44107.
