Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: Press #3: On, or about, August 23, 2022, an employee was seriously injured when the eye-bolt installed on the river-side bearing block failed under tension, causing the hook on the ratchet strap to fly back and strike the employee in the face. The eyebolt used was not rated for use where exposed to any significant amount of force.
