29 CFR 1910.23(c)(9): The employer did not ensure that portable ladders that are used on slippery surfaces are secured and stabilized. A) International Timber and Veneer on or about 08/23/22 at the rear of the chipper room area: An employee was injured while using a Werner Model D1340-2 extension ladder when the base of the ladder kicked out while the employee was approximately 10-feet above ground level. The paved area was slippery due to wet impurities from the water holding vats. The ladder was not secured prior to use. The feet/cleats on the ladder were not kept free of defects in that material was built-up in the feet and the feet were past their useful life and needed to be replaced.
