29 CFR 1926.453(a)(2): Aerial lifts were "field modified" for uses other than those intended by the manufacturer, the modification had not been certified in writing by the manufacturer or by any other equivalent entity. Modification did not conform with all applicable provisions of ANSI A92.2-1969: On about August 23, 2022, at the proximity of the NW 3rd Avenue near NW 14th Street in Miami , Florida, employees were exposed to struck-by and crushing hazards when two aerial lifts were used for a formwork removal operation which was not allowed by the equipment manufacturer.
