29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high. a) DOA Line 1 mid-landing; On or about August 24, 2022 and at times prior, operators and maintenance employees were exposed to fall hazards, in that the middle landing on the DOA did not contain a mid-rail exposing employees to a 5 feet 6 inches fall.
