A continuing and effective extermination program was not instituted where their presence is detected. a) Hospital Cafeteria Kitchen - On or about August 25, 2022, the employer exposed employees to health hazards in that, the employer failed to establish an effective vermin control plan and to ensure that every enclosed workplace was constructed as to prevent the entrance of rodents into workplace while employees were working in the Kitchen area and keeping their foods and beverages.
