29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. a) On or about 08/04/2022, the employer did not ensure that a work-related fatality for an employee under their supervision was reported within 8 hours to OSHA.
