29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence: Location Warehouse floor. On or about August 8th 2022, employees were exposed to struck-by and crushed-by hazards while performing hands-on training on an electric riding pallet jack where the trainer left them unsupervised.
