29 CFR 1910.146(c)(9)(i): Each contractor who was retained to perform permit space entry operations did not obtain any available information regarding permit space hazards and entry operations from the host employer: (a)On 8/15/2022 and at times earlier, in/at the Coke Hopper, contractor employees were exposed to falls of up to 13 feet to the bottom of the hopper, and hazardous energy inside the hopper at the point of operation for the vibrator because CORE, a contractor who was retained by ACIPCO to perform permit space entry operations, did not obtain any available information regarding permit space hazards and entry operations regarding the coke hopper from ACIPCO.
