29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) 400 E. Tioga St, Philadelphia, PA : A floor opening measuring approx. 19" wide by 17" deep over a sump pump pit was not covered or guarded, exposing employees to trip and fall hazards. Condition noted on or about the 6-month period of Feb. 12 - August 12, 2022.
