29 CFR 1910.23(d)(7): The employer did not ensure that grab bars extend 42 inches (1.1 m) above the access level or landing platforms served by the ladder: a) BAE Shipyard: On or about August 01, 2022, an employee required to access the top of a 20ft blast pot to operate a lever, was not provided with a grab bar at the top of the ladder. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
